Is an Open margin considered as a Defective Restoration? Because defective restoration need to be replaced but with open margins don't always need replacement.

The grade selected in this question was grade B. Shouldn't it be grade C as the patient is a heavy smoker?

In the Case patient BP is 130/85 the answer should be short appointments and minimize epi but in the video Dr. Ahmad said none of the above please explain
